Note: You will not be able to submit your student's enrollment without uploading the required documents, please have pictures already taken of the items as you will be uploading them as attachments. Please save them in the following formats only, .PNG, .JPEG, OR .PDF - any other formats and the item will not be accepted.
- 🏠 Most recent Mortgage Statement, most recent Sacramento Property Tax statement, Purchase Agreement or Rental/Lease Agreement - Must display parent/guardian name, residence address, current dates (or month-to-month continuation if lease). Agreements must also be signed by both Seller/Buyer or Landlord/Tenant.
- 💡 Most recent statement for one of the following: SMUD, PG & E or Water Utility - must contain parent/guardian name, residence address and current statement date.
- 🆔 Parent/Guardian Legal Photo Identification: CA Driver's License, ID, Military or Government Issued Passport - these items must not be expired.
- 📜 Student birth certificate
- 🩺 Medical or State record of Student Immunizations. See Required Immunizations
These additional items must be provided if applicable:
- Copy of Student IEP or 504 - (all pages are required, please obtain an electronic copy from your student's current school if you do not have one.)
- Shared Residence Affidavit - if parent/guardian and the student are living with another family and proofs of residence are not in their name
- Caregiver Affidavit - if the student is living with someone other than their parent/legal guardian
Please be aware that all enrollments under a shared residence/caregiver affidavit require admin approval and are verified by our school resource officer through home visits and other methods of address verification.
Note: Incoming transfer students who have a sibling, who is enrolled at AnHS for the 2025-2026 school year, are eligible to apply and have their applications reviewed.
